Paperwork Submission Guidelines

Overview

This document covers how Training Faculty (TF) are required to submit instructor course, instructor renewal, and instructor transfer paperwork.


Instructor Course Paperwork

  1. Before teaching an Instructor course, create an instructor course in Atlas.*

  2. Add students to the class roster.

  3. If class is not created in Atlas, paperwork will not be accepted until this is completed.

  4. Please wait for paperwork to be reviewed. Estimated time to complete is 4-5 business days.


This roster will be finalized when you complete all paperwork by Vitali. After paperwork approval, your students will be issued instructor certifications.

Instructor Paperwork

You will submit instructor paperwork using the Instructor Intake Form. Please follow the directions carefully:

  • Submit paperwork for each instructor as a .PDF file.

  • Submit only ONE .pdf file per instructor.

  • Pictures (.jpg) or separate files of each piece of paper will not be accepted

  • See file formats below for paperwork order for: new instructor paperwork, renewing instructor paperwork, transferring instructor paperwork, & secondary alignment only paperwork.

PDF Tips:

  • You can use a scanner app on your mobile device to create PDFs. 

  • If you have multiple .PDF files you must convert them into one file using an online file converter.

  • If the .PDF file is too large to upload, go to https://www.compress2go.com 

    • Select “Compress PDF”. 

    • Upload the PDF file you want to compress

    • Select “Strong Compression” 

    • When the PDF is fully loaded and the bar turns green, select “Start”. 

    • When done, click “Download”

  • If this does not compress the file small enough, please reach out.


Instructor Alignment

After submitting instructor paperwork, follow the Aligning Directly Aligned Instructor Under TS to align the instructor under your Training Site.


New Instructor Paperwork

Name the file in this format: “Instructor Course_[discipline]_[instructor name].pdf”

  1. Instructor Candidate Application 

  2. Copy of Provider card

  3. Instructor Essentials Certificate

  4. Instructor Essentials Test

  5. Monitoring Form

  6. AHA Affiliation Agreement


Renewing Instructor Paperwork

Name the file in this format: “Renewal [year]_[discipline]_[instructor name].pdf”

Transferring Instructor Paperwork (For Primary Alignment Only)

Name the file in this format: “Transfer_[discipline]_[instructor name].pdf”

  1. Transfer form

  2. Monitoring form


Secondary Alignment Only

For instructors who help you teach classes but are primarily teaching under another TC.

  1. Copy of a current Instructor card(s)
